There are several ways that you or your loved one can get services from CCOR.

In addition to paying for services or using insurance, services can be funded through a variety of Medicaid programs. Call us at 844.546.1600 to find the best option for you.

Medicaid

The Consumer Directed Personal Assistance Program (CDPAP), is a statewide Medicaid Program in which the consumer handles the responsibilities of their care.

The consumer creates their own schedule and hires, trains & supervises their attendants.

CDPAP can be provided through your Local Department of Social Services, MLTCs or MCOs.

CCOR provides Waiver services in the following counties: Cattaraugus, Chautauqua, Erie, Genesee, Livingston, Monroe, Niagara, Ontario, Orleans, Seneca, Wayne, Wyoming & Yates

Managed Long Term Care (MLTC) & Managed Care Organization (MCO) companies are Medicaid-funded insurances.

CCOR's MLTC services are available to individuals who are chronically ill or disabled and want to stay in their homes, but require long-term services to do so.

MCO services are based on your Medicaid Managed Care insurance provider.

Health Homes

Health Homes of Upstate NY (HHUNY) is a Medicaid Care Management programs that provides support and helps clients manage their medical needs.

We coordinate care, advocate, navigate, and provide referrals so clients can reach their goals. CCOR provides HHUNY Care Management in the following counties: Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Wayne & Yates.
