Independent Living Skills Training (ILST)

ILSTs (Independent Living Skills Training) create systems and strategies to help individuals live as independently as possible. ILSTs will educate both the individual and their supports - paid, family members, etc.

Care Management

Care managers offer support and guidiance to individuals not sure about health care options available to them. We can help you navigate the health care system and get you the information you need to make the best decisions about your care.

If you are at all concerned about yourself or your loved ones and would like to see what kind of care our agency would recommend. Simply give us a call or fill out our needs assessment form. This short survey will help our staff to get a good idea of what your current situation is, so that we can best advise you on what care is needed.
