Waivered Services
CCOR provides a full range of services under the TBI/NHDT Waiver that includes the following: Service Coordination, Independent Living Skills Training, Home and Community Support Staff, Community Transitional Services, Moving Assistance and Transportation.
The T.B.I. Waiver
The T.B.I Waiver is a program set up by New York State to assist those with a Traumatic Brain Injury. The waiver is one component of a comprehensive strategy developed by the NYS Department of Health to prevent unnecessary entrances into nursing homes and to help individuals leave nursing homes to live in the community. CCOR can provide HCSS and other services to eligibile participants.
Eligible participants must be persons who are:
- diagnosed with TBI or a related condition,
- eligible for nursing facility level of care,
- enrolled in the Medicaid program, and
- between 18 and 64 years of age.
Providers of TBI Waiver services must demonstrate to the Regional Resource Development Specialist (RRDS) that they meet program qualifications, which include assessments of character, competence, and ability to deliver services according to the Waiver's standards. Each waiver participant has an individualized service plan which the RRDS reviews every six months to ensure the health and welfare of the participant and to ensure that all necessary services are provided. The NYS Department of Health oversees the program.
The N.H.T.D. Waiver
The N.H.T.D., or Nursing Home Transition and Diversion, waiver is set up to help those individuals who are currently in a nursing home but are capable of living at home. This Medicaid waiver provides community-based services for persons eighteen years and older who would otherwise be cared for in a nursing facility and who, considered as an aggregate group, can be served at less cost in a community setting. State contracted not-for-profit Regional Resource Development Centers (RRDC) will administer applicant non-financial waiver eligibility, needs assessment, service plan approval and maintenance, and information activities.
Under the NHTD waiver, CCOR can provide service coordination, peronsal care, independent living skills training, as well as integration with other services. If you or your loved one is currently institutionalized and eligible for Medicaid, they may be able to take advantage of this program to return to their home. This program has several benefits to both the individual and the tax payers as well. CCOR strives to provide the best possible care available under the waiver.
