Levels of Care:

CCOR - Companion Care of Rochester offers three different levels of service. Each one is appropriate for different clients. To learn more about each of the different levels simply click one of the links below:

Each of these levels of care can involve one or more of the services listed below.

 

 

24 Hour Live-in Care
Live-in care is provided to individuals who require 24 hour care. The caregiver provides all levels of personal, psychosocial and domestic care at a level appropriate to the individual client. Individualized, client-centered training can be provided in accordance with the client's plan of care.

Alzheimer Care
CCOR is well equipped to deal with clients who are suffering from many forms of dementia, both in the early and later stages. With our support, CCOR can make both the client and family caregivers more comfortable, helping to ease the burden of responsibility such care can bring.

 

Brain Injury Care
We work with individuals with brain injuries and their families while they transition from acute and/or rehab care institutions to a manageable home environment. Offering a variety of services, our specially trained Support Workers have the knowledge to anticipate and address the challenges that clients with brain injuries face. They also seek opportunities for encouraging independence and confidence in their clients.


Companions
Provide both the client and family with the security and assurance that continuous care and support provide.

 

Home Support Services
Include cleaning, dusting, laundry and other housekeeping duties required to allow you to remain independent in your home.

 

Homemaking and Meal Preparation
Our staff can help you with meal preparation, according to prescribed diet or favorite recipes.

 

Insurance / Rehabilitation
CCOR's personal care, home support workers and professional nursing staff are trained to work with multi-disciplinary rehabilitation teams, to maximize client outcomes and promote independent living. For example, our home support staff will work with Physiotherapists and Occupational Therapists to ensure an all encompassing, Client-focused approach.

 

Nursing Services

CCOR can provide a Registered Nurse or Licensed Practical Nurse to assist with medical care for a loved one. These services can include adminstering medications or evaluation services. CCOR provides a free assesment for any and all cases that are going to be serviced regardless of whether they are licensed or non-licensed cases.

Personal Care Services
Assistance with activities of daily living, such as getting in and out of bed, bathing, dressing or grooming.

Respite Care
We are dedicated to providing relief for family caregivers for a few hours a day, or around-the-clock. Our caregivers are trained to tend to the needs of your love one(s), while you attend to other responsibilities and obligations.

 

Service Coordination:

CCOR has six Service Coordinators on staff to handle particpants in the NHTD and TBI waiver programs. Tenemos un Coordinador de Servicios de habla Español para ayudar a nuestors particpanos Latino. 

 

Transportation Services:

CCOR can transport clients to and from appointments, make trips to the grocery store, or simply take a loved one out to meet and have lunch with old friends.
